They give you a "fake medal" which helps to unlock tracks but it doesn't count towards trophies. For example- a joker bronze won't unlock the "all bronze" trophy. You get the jokers by failing to get the time above several times. So to get a fake gold- you have to get a silver time a few times in a row.
