Having ESO+ gives you access to all DLCs, but not all chapters. For example, Northern Elsweyr is a chapter, but Southern Elsweyr is a DLC. Greymoor with Western Skyrim is a chapter; and when Markarth comes out with the Reach zone that will be DLC. Chapters have to be purchased separately.
